Yesuna Community Care ensures participants have full control over their support providers and how support is delivered while maintaining relationships with service providers and monitoring NDIS budgets for continuity of care. We also assist participants in preparing for NDIS reviews and facilitate plan modifications when needed.

Understanding Your Level 2 Support Coordination

At Yesuna Community Care, our Level 2 Coordination of Support service is dedicated to helping NDIS participants effectively understand and implement their NDIS Plans. Upholding the highest NDIS standards, we ensure that participants have full control over their support providers and the manner in which support is delivered. Our coordinators also play a crucial role in maintaining relationships with service providers and monitoring NDIS budgets to ensure continuity of care. Regular reporting to the NDIA and assistance preparing for NDIS reviews are integral to our service. Additionally, we understand that unforeseen events may necessitate changes in a participant’s goals and needs during the plan period. In such cases, our Support Coordinators guide participants through the NDIS process to modify their plans seamlessly, facilitating linkages with necessary assessments, gathering essential documentation, and applying for reviews with the Agency.
